| | | | | | | | | I. | MOTION REQUESTED | | | A) Approve Amendment No. 4 with JSM Services, Inc., under Solicitation No. RFP180313KLC, Athletic Turf Maintenance and Reconstruction, authorizing a Consumer Price Index (CPI) increase of 4% for the price of athletic turf maintenance and reconstruction services, as provided in the amended Fee Schedule.
B) Authorize the County Manager or designee to execute the amendment on behalf of the Board of County Commissioners.

| | | | | | | | | | A) | Board Action and Other History | | | | On November 20, 2018, the Board of County Commissioners awarded RFP180313KLC, Athletic Turf Maintenance and Reconstruction - Annual, to JSM Services, Inc. to provide athletic turf maintenance and reconstruction services. The original term of the contract was for 2 years with an option to renew for 3 additional, 1 year periods.
On September 20, 2019, Amendment No. 1 was administratively approved to add the Exhibit D. Vendor Background Screening Affidavit, requiring JSM Services, Inc. comply with statutory background screening requirements.
On January 10, 2020, Amendment No. 2 was administratively approved to increase the service area for the North Fort Myers Academy of the Arts under Section 7. Turf Maintenance of the Exhibit A. Scope of Services of the contract.
On October 14, 2021, Amendment No. 3 was signed by JSM Services to decrease the size of the athletic turf maintenance and reconstruction service area, at Estero High School location, add additional service locations for South Fort Myers High School, Fort Myers High School, and Dunbar High School, and remove the City of Palms Park and Schandler Hall service locations under Section 7. Turf Maintenance of the Exhibit A. Scope of Services of the contract.
